DRAFT FOR REVIEW
This is a starting draft written for the practice as described, not legal advice. Have an attorney licensed in New York review it — and confirm the HIPAA and telehealth language against your actual systems — before publishing.
Questions about this policy or about information held about you: sherrell@smoothtransitionsllc.org
Smooth Transitions LLC ("Smooth Transitions," "we," or "I") respects your privacy. This policy explains what information this website collects, how it is used, and the choices you have. It covers smoothtransitionsllc.org only — clinical records for counseling clients are governed separately by the Notice of Privacy Practices provided at intake.
01
Information collected
Information you give me. Your name, email address, phone number, organization, and whatever you choose to write when you email me, submit an inquiry, or book an appointment.
Scheduling and clinical platforms. Coaching and organizational calls are booked through Calendly; counseling scheduling, documentation, and payment run through SimplePractice, a HIPAA-compliant electronic health record. Information you enter into those platforms is held under their own terms and privacy policies.
Technical information. Standard server and analytics data — browser type, device, referring page, approximate location, and pages viewed — collected automatically to keep the site working and understand what people find useful.
02
How information is used
To respond to your inquiry, schedule and deliver services, send information you requested, process payment, meet legal and professional obligations, and improve the site. I do not sell your information, and I do not use it for third-party advertising.
03
Protected health information
Counseling is a licensed clinical service, and clinical records are protected health information under HIPAA and New York law. Those records live in SimplePractice, not on this website, and are governed by the Notice of Privacy Practices you receive at intake. Please don't send clinical details, diagnoses, or crisis information through this website or by email — email is not a secure channel and is not monitored around the clock.
04
When information is shared
Only with service providers who help operate the practice — scheduling, records, payment processing, email, and website hosting — and only as needed to provide the service; when you ask me to; or when the law or my professional obligations as a Licensed Clinical Social Worker require it, including situations involving risk of serious harm or mandated reporting.
05
Cookies and analytics
This site uses cookies and similar technologies for basic functionality and aggregate analytics. Embedded scheduling widgets may set their own cookies. You can block or delete cookies in your browser settings; some features may not work as well if you do.
06
Retention and security
Inquiries and correspondence are kept as long as needed to respond and to meet business and legal requirements; clinical records are retained for the period required by New York law and professional standards. I use reasonable safeguards to protect your information, but no method of transmission or storage is completely secure.
07
Your choices
You can ask what information I hold about you, ask that it be corrected or deleted, and unsubscribe from any non-essential email at any time. Some information must be retained where law or professional obligation requires it. Depending on where you live, you may have additional rights under state privacy law.
08
Children
Services are for adults 18 and older, and this site is not directed to children. I do not knowingly collect information from anyone under 18. Group work with high school students is arranged through the organization hosting it, with consent handled there.
09
Third-party links
This site links to platforms I use, including Calendly and SimplePractice. Their privacy practices are their own; please review their policies before entering information.

Changes and contact
If this policy changes, the effective date above will be updated. For questions, or to make a request about your information, email sherrell@smoothtransitionsllc.org.
